The ADC is recognizing National Healthcare Decisions Day on April 19th with a drive through at the ADC.
Stop by between 2:00 p.m. and 3:30 p.m. and receive a copy of the Five Wishes, meet Robin Williams of Hospice of NJ, and enjoy a snack with the ADC team.

Five Wishes is America’s most popular living will because it is written in everyday language and it helps people express their wishes where it matters most. It includes considerations for personal and spiritual preferences in addition to medical and legal issues. It is also a blueprint for describing what good care means to you and detailing to your family exactly what you want.

Robin will be available to answer questions on April 19th and we are planning a follow up question and answer session for early in May.

Please note: NHDD is officially scheduled for April 16th but for our purposes we are making plans for April 19th.
