As a reminder, the ADC will be closed on Tuesday, December 24th and Wednesday, December 25th in recognition of the Christmas Holiday. The Application Process

The Application

You may call us to request an application or download/print HERE. 

There are four parts to the application:

  • Enrollment forms (to be filled out by you)
  • Financial Disclosure (to be filled out by you)

    We ask for you to fill out the financial information, to help us determine whether you qualify for financial assistance or our sliding scale.

  • COVID-19 Disclosure Forms (to be filled out by you)
  • Medical History (to be filled out by your loved one's physician)
  • Schedule a tour

    Either prior to submitting the application or after submitting the application, we suggest you arrange for a site tour with our social services coordinator, Stacy Scelfo. We want you to be comfortable with the program before you send your loved one to us.

  • Home Visit

    Once Stacy receives your application, she will contact you to schedule an at-home assessment, during which she will describe the next steps in the application process. She can be reached at (908) 725-0068 or sscelfo@adultdaycenter.org.

    At the home assessment, Stacy will work with you to pick a mutually convenient start date.

Please note, at present, it may take approximately 4-5 weeks for a new member to start after submitting a completed application packet.

As new members join the ADC, we want them to get acclimated and our staff to come familiar with their needs. For this reason, we don’t start too many new members at once. 

We will do everything to accommodate your family member in as short a time as possible.
